· Download and complete E-passport application form

· Current Thai passport, original or its copy

         · Thai house/Temple registration, original or its copy

         · Identity booklet for a Buddhist monk, original or its copy

         · An official letter from Sangha Supreme Council, Office of National Buddhism. For Buddhist monk who

        has been ordained in Australia, an official letter from the abbot of his temple of residence can be substituted.
